Resident-led Inter-professional Team Launches Heart Failure Clinic in the IMC

Starting in January 2017 a resident team has initiated disease based visits in the IMC for patients with a diagnosis of heart failure. The goal is to move this population forward in their use of current HF protocol and to reduce hospital readmissions.

 

During the visit, patients are seen by the pharmacist, behavioral health and the managing resident. Visits are prepped using a special template that is added to the Care Coordination Note and the visit itself follows HF guidelines using a workflow and note template.  Use the PCMH menu item to access the HF Page.

 

Interns Join the Internal Medicine Center Practice

We are excited to welcome the 2016 interns to our program and to the Internal Medicine Center practice. Even though you may not rotate through until later in the year for your dedicated ambulatory month, all our interns will receive a vigorous training for our EHR (a version of EPIC called CarePath) and an orientation to the staff and patient care at the IMC. Stop by and let us know you are here. Everyone is ready to assist you in learning to see patients in the ambulatory setting as the newest members of our team.

When and Where?

Didactic Cases are specially written to introduce you to the key elements of the diagnosis and treatment of 10-12 of the most common conditions you are likely to see in the Internal Medicine Center practice. The cases include a real life presentation for the condition with questions that lead you through all relevant issues of diagnosis and treatment. Links are provided to critical and current evidence-based guidelines

This page contains a calendar and links to access all cases. If you are the lead or back up presenter, be prepared to read the case and guide the group through answering the questions.

Even if you are not leading, work through all the questions ahead of the session using the guideline links provided.

All Didactic Sessions are held in the Bullpen, or staffing area, in the IMC. Sessions start promptly at 8:00 so plan to be settled and ready to start  before then.
